Services will be private. National Cremation and Burial Society is in charge of arrangements.
Survivors include his wife of 33 years, Barbara Jo; children, Anthony DeVito, Nicki See, both of Indiana and Gina Capps of Nevada; sister, Esther Cox of Liberty, Indiana' 6 grandchildren and 4 great grandchildren.
He is preceded in death by his son, John Ronald.
Jack owned and operated Burkhardt Tax Service and Pronto Home Improvements Service in Richmond, Indiana. He retired to Sarasota where he was proud to work 17 years with Publix, up until he was 94.
In lieu of flowers, donations may be made to American Cancer Society in Jacks memory.
