Richard G. Samolyk, Born to Eternal Life October 10, 2012 age 49 years. Beloved son of Martha (nee Woyach) and the late Richard L. Samolyk. Dear brother of David (Lori). Dear uncle of Sara (Aaron) Rostad. Also survived by aunts, uncles, cousins, other relatives and friends.
Funeral Mass Tuesday, October 16, 11 am at Our Lady of Divine Providence Parish-St. Mary of Czestochowa Church, 3055 N. Fratney St. Procession to Holy Cross Cemetery after Mass for chapel committal. Visitation Tuesday at the Church from 9:30 am until time of Mass. The family would like to acknowledge the care and services of Golden Living-Colonial Manor and Heartland Hospice during the last three months of Rick's journey.
