Jerry P. DIAZ, age 79, of Chicago, Illinois passed away on Friday, March 25, 2022. Jerry was born October 12, 1942. Beloved son of the late Jesse and Rosalie M. Diaz, beloved brother of the late Patricia (the late John) Jaszka, the late Frank (the late Sophia), Robert, Mary Ann (Richard) Drabik, Susan Diaz and Kathy Mondala (Eugene Sieczkowski), beloved uncle to many nieces and nephews.
A visitation for Jerry will be held Friday, April 1, 2022 from 9:00 AM to 11:00 AM at Malec & Sons Funeral Home, 6000 North Milwaukee Ave, Chicago, IL 60646. A mass of christian burial will occur Friday, April 1, 2022 from 12:00 PM to 1:00 PM at St. Elizabeth of the Trinity, 6020 W Ardmore Ave, Chicago, IL 60646. A committal & internment will occur Friday, April 1, 2022 from 1:00 PM to 2:00 PM at St. Adalbert Cemetery, 6800 N. Milwaukee Ave, Niles, IL 60714.
