He is survived by his son, Dan (Kay) and daughter, Janice Lamberton. He is also survived by two grandchildren, Travis (Tracy) Underwood, Tracey (Joseph) Novotni; two great-grandchildren, J.T. Novotni, Jake Novotni; and sister Kathleen (Underwood) Dickens.
In addition to his parents, he is preceded in death by his wife E. Madeline (Burton) Underwood of 75 years and his siblings, Bernice (Underwood) Engle, May Underwood (infant), Eugene Underwood, Marjorie (Underwood) Smith, Harold Underwood and Patricia (Underwood) Collins.
Jack served in the Army with the Corps of Military Police and was honorably discharged in 1948 after World War II. He retired from Armco Steel in Middletown OH.
