Hazel was born August 23, 1921 in Philadelphia, PA, daughter of Grace Jeffries Adams and Noel Lafayette Adams. She married Harold Elmer Cook on June 1, 1944. She died June 4, 2012. She is survived by her daughters, Judith (and Millard) Williams of Oskaloosa, IA and Susan (and Edward) Upshaw of Chesterfield, VA; 5 grandchildren; and 11 great-grandchildren. She was preceded in death by her husband and her brother, Gerald L. Adams. According to her wishes, she was cremated and her ashes placed in the Florida National Cemetery, beside her husband.
