

He was born May 1, 1929 in Kingston, NY, the son of Duane J. and Ethel M. (Clayton) Forman. He married Charlotte I. Decker in Catskill, NY on May 15, 1955. Charlotte predeceased him on October 29, 1994.
John attended elementary and high school in Kingston and graduated from Albany College of Pharmacy in 1952. He practiced Pharmacy in Kingston and Catskill, NY. He worked for General Motors “Parade of Progress,” and for Merck & Co. before coming to Oneonta in 1961, where he operated “Medical Arts Pharmacy” in partnership with Frank H. Moak until 1990. John was a member of the First United Presbyterian Church where he served as an elder.
John served during WWII with the 24th Infantry Division attaining the rank of Sergeant. He was a life member of the Oneonta American Legion Post 259 and the V.F.W. post 1206. He was a 30 year member of the Board of Directors of the Oneonta Veterans Club and a member of the 24th Infantry Division Association.
He was a member, past president and member of the board of directors of the Tri-Valley Pharmaceutical Association and a member of the National Community Pharmacists Association.
He was a life member of the Oneonta Elks Lodge 1312, a member of the 6th Ward Athletic Association, a longtime member of the Oneonta Country Club and a member of the Board of Directors on the Glenwood Cemetery Association.
John was a longtime Mason, member of the Kingston Lodge No. 10 and past member of Oneonta Lodge No. 466 F&AM. He was a 32nd degree Scottish Rite Mason, member of the Council, Chapter and Consistory bodies in the Valley of Binghamton and past member of the Kalurah Temple Shrine.
John’s passion was his children, grandchildren and especially his great granddaughter, Ella who brought John great joy. John also loved golfing with his friends.
John is survived by his two daughters, Patricia Forman of Oneonta and Susan Gonzalez of Grahamsville NY; four grandsons, Ramon III and his wife, Suzanne of Slate Hill, Daniel and his wife Erica of Forestburgh, Marc Gonzalez of Forestburgh and David Gonzalez of Goshen; great granddaughter, Ella Gonzalez; as well as several nieces, nephews and cousins.
Memorial services will be held on Wednesday, June 30, 2021 at 1:00 PM at the First United Presbyterian Church “Red Door” with the Rev. Cynthia Walton Leavitt officiating.
