So sorry to hear about Dr Hoffman's passing... just learned about it today.  He was one of my doctors before I moved out to California... and he passed shortly after I moved.  He was a good man, and a fantastic doctor.

I'm very sorry to hear of Dr Hoffman's passing.  I lost my mom about the same time and I hadn't heard of his passing.  He was our family doctor from 1984 till his retirement.  My mom was his patient as well.  Prayers for his close friends and family!

Dr. Hoffman came out to our business and gave flu shots to my employees every year for 20+ years.  He was a good man.  How many doctors would do this? None, except him.  He will be missed.

I just found out through a co-worker of his passing. I was a patient of Dr. Hoffman's for about 7 years and he was always caring and kind. May he find peace and rest. My deepest condolences regarding his passing : (

This man worked as a Dr with an office and doing hospital rounds for 6 days a week continuously. So sad he won't get to enjoy his golden years enjoying a planned retirement. Dr Hoffman saved my life. He's also the only Dr who would actually get on the phone with me.  He was such a character and did so much for children all over the area have Christmas Toys every year. You will be missed Dr, in our prayers. 

Dr.Hoffman was not only a Great Dr. he was also a great friend for many years.  He was also both of my boy's Secret Santa for many years ( oldest being 35)... I will miss you my dear friend and many Blessings for everything Positive you brought to my life!!! Til we meet again,  Always in my heart and never far away... Another Guardian Angel to watch over us !!!💜🖤💜

I met Doc Hoffman just over 5yrs ago when I moved back to IL.  Friends of mine suggested him and had been patients of his for years.  I even saw him once when friend was in hospital and doc came by OSF after work and checked in on his patients before going home for the night, and start doing the same thing next day.  He was a great doctor for the short 5 1/2 years I had him.  He always had toys for the kids in his office around the holidays especially!!  He was one of the best around in my book and I was shocked to hear he was retiring.  Even worse when I heard today that he had passed.  Thank you Dr Hoffman for everything you did and have done for all your patients and I consider you one of the BEST doctors I ever knew.  Rest easy now and I will miss you 😢 

Dnd and I were in med school together. We worked together for four years, partied together and stayed in touch long after grduation.  Later in life we shared interests and always sent Christmas gifts. Both my wife and I are so sad to learn of his passing. 

Dr Hoffman has been my Dr since he started in Dunlap.
He was a excellent doctor.He will forever be missed.
He has saved Caryls life on at least two occasions by his quick action and attention.
You will always be missed.We will meet again.

Dan and I were in a group that watched Monday night football. I dont think that he was a football person but really enjoyed being a part of the group.AT Christmas he bring present for everyone. I believe that he was of the Jewish faith. Yes Dan was number One in my book. H had saved one our group's  life by getting him into a heart specialist just in time

Dr Hoffman, was my doctor for over 10 years and I always felt he was my friend too.

Dr. Hoffman was our family doctor in the 1970s and 1980s. He was a very caring person and was sweet and gentle with my daughters. Could not have asked for a better doctor.

Dr Dan was a very special kind of Dr..
Years ago , I was in the hospital, under a surgeon's care. He walked by my room , saw me, and stopped in to say hello..he was always so busy.

He had arcade games. With free tokens. In his waiting room , where the kids would play, happily
He had a pirates treasure chest, full of toys as well for the little people , such a blessing to watch the kids dig thru to find the right toy.   That was a big decision..

He had a special room where he kept supplies and he bought toys for the  Toys for Tots program,  each year..

Most importantly he would take the time to talk with you, and if you needed, he shared free samples. 

I remember I had just got a shot, walked out to receptionist desk. I slid down the wall and fainted..
He sat me down , in his chair , in his office until I recovered... He had deep concern for his patients and office staff,.. a good person 

Dr. Hoffman was my Doctor. He was awesome and kind and funny. I will remember him always. Will miss him.

Sorry for your loss; my heart breaks at losing one of the best doctors in this area! Our first encounter with Dr Hoffman was when my son at the age of 10 was in scouts and needed a physical for an outing adventure to Canada. The second encounter a few years later my daughter who was 14 was very ill came to ER at 3 am and that’s when I decided that’s the kind of doctor I wanted . He’s been by doctor for over 20 years … this was the hardest change ever when I received the call about him retiring and having to find a “new” doctor  - oh no this isn’t happening???? I know he will be missed and I’m proud to say he took great care of me each time I was hospitalized and that I called him “My Doctor!” RIP - Doctor Daniel Hoffman
