Trevor Lee Koontz, our beloved brother, son, father and friend Passed away on the evening of November 13, 2012 in the same hospital where he was born 38 years earlier. He had his family by his side, holding his hands.
Trevor was a lifetime resident of Ventura, born here on May 14, 1974. He was so talented, intelligent and loving. He will be missed so much by all of us.
He is survived by his 3 sons, Levi, Trevor and Ryder. His sister, Trisha, brother Joshua, parents Larry and Telva and nephews, aunts, uncles and many cousins.
A service to celebrate him and his life will be held at Charles Carroll Funeral Home 15 Teloma Dr. Ventura, CA 93003 on Sunday November 18th at 11am.
