Kaye Ellen Formaro, 73, of Cumming, GA formerly of Des Moines, passed away on Saturday, July 23, 2016 at Sunrise at Ivey Ridge in Alpharetta, GA. A visitation will be held from 1:30 to 2:30 p.m. Friday, July 29, 2016 at McLaren’s Chapel in West Des Moines with the burial following at Resthaven Cemetery in West Des Moines.
Kaye was born December 18, 1942 to George and Doris Hukill. She married Charles R. Formaro and had three children.
Kaye is survived by her sons, Charles (Lisa) Formaro Jr. of Cumming, GA, Robin Formaro of Alpharetta, GA and Randy Formaro of Doraville, GA; grandsons, Nicholas, Mason and Joseph; granddaughter, Kelsey; and many nieces, nephews, cousins and friends. She was preceded in death by her parents; husband, Chuck; and sister, Vicki.
In lieu of flowers, memorial contributions may be made to Alzheimer’s Association-Greater Iowa Chapter.
