Takeo “Tak” Azuma passed away April 24, 2013 peacefully at home. Tak is survived by his wife Shizuka “Susie” Azuma; sons Neil (Lynne) Azuma, Dale (Pamela) Azuma and daughters Donna (Dennis) Chock, Kay (Robert) Chavez and eight grandchildren.
A private family service is to be held at a future date. In lieu of flowers or Koden please send donations to Gentiva Hospice.
