She was born on February 22, 1941in Riverhead, NY, to Frank and Wanda Cybulski Perkowski.
She attended Riverhead HS and was known as Connie to all her friends and family. She was married September 27, 1958 in St. John’s church in Riverhead to Phillip R. Smith of Huntington, WV. In her spare time, she loved arts & crafts, if she wasn’t keeping in touch with her friends and relatives.
She was preceded in death by her parents, sisters Gloria Perkowski Brown and Pauline Woschnik, her husband Phillip R. Smith and her grandson, Phillip R. Smith, III (age 6), who was her pride and joy.
She is survived by her son, Phillip R. Smith, Jr. and his wife Linda Smith of Canton, TX, daughter Diane E. Smith of Lancaster, TX, granddaughter Janis Wilbert of Dallas, TX, Janis’ husband Peter and their two daughters Allison and Emma, as well as her brother Allen Perkowski, sister Marion Kruk and nieces and nephews.
Internment is at Haven of Memories Memorial Park in Canton, TX, where she requested private services.
