

Claude R. Poliak, M.D. of White died Thursday, October 27 2011 at Cartersville Medical Center. He was a native of Massachusetts and resided in many areas during the course of his military and professional careers. He attended high school in France and graduated from the University of Medicine and Dentistry of New Jersey. He served in the U.S. Air Force during the Viet Nam era as a Flight Surgeon/ophthalmologist and retired from the U.S. Navy as a Commander. He also had private practices in Tampa, Florida and Cartersville, Ga.
Claude is survived by his wife of 33 years, Diana Poliak, his daughter Juliana Helene Poliak, and extended family in New England and France. He was preceded in death by his father Mendel Poliak, M.D. and mother, Blanche Bariston Poliak. He served his community through many civic and medical services. He was an associate professor at the University of South Florida and published journal articles. Dr. Poliak was a member of numerous professional, civic and military organizations and was past Worshipful Master of Cartersville Masonic Lodge #63.
He had a love of family and friends with an exuberant sense of humor and will be missed by many.
Funeral services will be held 2:00 P.M. Saturday, November 5, 2011 from the Chapel of the Owen Funeral Home with Rev. Mark Woodruff officiating. Interment services will be held at a later date this year at Arlington National Cemetery, in Arlington, Virginia with full military rites observed.

The family will receive friends at the funeral home from 12:00 till 2:00 P.M. Saturday, November 5, 2011.
