Phyllis Jean Carmack passed away on Thursday, May 19, 2016. A funeral service will be held on Friday, June 3, 2016 at Two O'clock in the Afternoon at OIinger Chapel Hill Mortuary, 6601 S Colorado Blvd, Centennial, CO 80121. She will be laid to rest at Olinger Chapel Hill Cemetery.
