

She was born on August 26, 1959 in Forsyth County to Miles (M.Z) and Ailene Collins of Winston Salem, North Carolina. She was preceded in death by her paternal grandparents, Zeno and Annie Collins, and her maternal grandparents, Fred and Ruth Evans both of Jonesville, North Carolina.
Tammy retired at the age of 51, with 30 years of service, from the Forsyth County Clerk of Court. Many of her colleagues were beloved friends whom she celebrated her life with. After she retired, she enjoyed spending time with her family, especially her grandchildren and nieces and nephews. Tammy was a follower of our Lord and Savior, Jesus Christ. She touched the lives of all those who knew her with her magnetic personality.
She is survived by her husband, Johnny Mathis of the home; three children, Jason Collins and wife Brandi, Eric Mathis, Ashley Warlick and husband Justin. Her siblings are Jeff Collins and wife Mary, Beverly Smith and husband, Jerry. The grandchildren are Dustyn, Haleigh and Jaxon Collins, Jonah Warlick, Jeana and Peyton Thomason.
A funeral service will be held at 3pm Saturday, with Rev. Billy Dickerson officiating at Frank Vogler and Sons Funeral Chapel, 2951 Reynolda Road. The family will receive friends from 1 until 3 PM, prior to the service, at the funeral home. Burial will follow at Forsyth Memorial Park.
