Jack is survived by his wife, Charlotte Collins; his daughter Shelia (Jim) Flood; and his son Greg (Yolanda) Collins; stepchildren Carla Miller and Patrick (Amanda) McQuaig.

9 Grandchildren and 8 Great Grandchildren

Jack was preceded in death by his parents Otto & Gertrude Collins; mother of his children Peggy Collins; grandson James Collins (Jay) Flood; brothers, Bud Collins and Robert Collins.

A celebration of life visitation, for Jack will be held Wednesday, July 19, 2023 from 1:00 PM to 2:00 PM at Northside Baptist Church, Cullman, AL, followed by a celebration of life funeral service at 2:00 PM.

Jack will be laid to rest in Bethany Baptist Cemetery, 1612 Co Rd 201, Crane Hill, AL 35053.
