Chad Martin Hornbrook, age 48, of Angola, Indiana, passed away on Sunday, February 4, 2024 with Jo Hornbrook Lyde and Jackie Godfrey by his side while at Northern Lakes Nursing and Rehabilitation Center, Angola, Indiana, under the care of Heart To Heart Hospice.
He was born on December 22, 1975 at Cameron Hospital, Angola, Indiana, and was raised by his grandparents.
Chad was a kind gentle soul. He was sweet and happy almost all the time, and would almost always smile. He was very smart, was a great judge of character, enjoyed being a prangster, and loved going for car rides.
Survivors include, his biological mother, Jo Hornbrook Lyde of Pensacola, Florida, aunt, Jackie Godfrey of Lake James, Fremont, Indiana, and uncle, John (Linda) Hornbrook, III of Bremerton, Washington.
He was preceded in death by his grandparents to whom raised him, John H. Hornbrook, Jr. and Thelma J. (Wilcox) Hornbrook, his Uncle Bob Godfrey, and Bob Lyde.
A gathering of family and friends to remember Chad and share memories will be held from 1 pm until 6 pm. Wednesday, February 7, 2024 at the home of Jackie Godfrey, 4980 N 300 W # 9, Landings Condominium, Fremont, Indiana 46737.
