56, of Glendale AZ passed away on October 16th, 2013. Carl was born in Phoenix AZ on March 30th, 1957 to parents Stanley Sr and Janet Tax. He attended Washington High School in Phoenix and later began a career with the city of Phoenix. In 1986, he married his sweet heart Kathy (deceased) and a son Daniel was born in 1987. Carl is survived by son Daniel of Glendale AZ, Brother Stan (Denise) of Sedona AZ, Brother Tim (Sherri) of Phoenix, sister Kim Tax of Phoenix, sister in law Mary Louise Cherpak of Glendale, brother in law John Cherpak of Glendale and numerous nieces, nephews, cousins and friends. A viewing will be held Wednesday, October 23rd, 2013 between 6:00 and 8:00pm at West Resthaven Funeral Home 6450 W Northern Ave, Glendale AZ 85301. Funeral services will be held Thursday, October 24th, 2013 at 10:30am at West Resthaven Funeral Home followed by a brief graveside services. In lieu of flowers a contribution can be made to Hospice of the Valley in Carl's Name.
