Daniel Wayne Smith was born February 15, 1939 in Dayton, Kentucky. He was born to Evelyn and August Smith. Daniel passed away October 27, 2015 in Portland, Oregon. He is survived by his wife Carol Smith, children Jeff Smith (Mary) and Tracy Hodge (William), sister Cheryl Rich, Aunt Dorothy Racke, and granddaughters Kaylee Smith, Annika Smith, Lindsay Smith, Justine Hodge, and Larissa Hodge.

The visitation for Daniel will be November 9, 2015 at 10:00-11:00am at Chapel of the Chimes Mortuary, 7924 N. 59th Ave, Glendale, AZ 85301, with services to follow.
