Janet Conley Polk 83 of Glendale passed away on Friday, February 13, 2015 at her home. Janet was a well-known and respected nurse and teacher for over forty years in obstetrics. She is survived by her daughters, Mindy Tuzzolino, Betsy Polk, Kathi Hutchins; a son, Larry Polk; a sister, Becky. She was a wonderful Grammy of six grandchildren, Nichole, Micah, Matt, Ryan, Tasha, and Cody and eight great grandchildren, Gabriella, Phelan, Jordan, Savanna, Kian, Kardan, Jade, and Greyson. The family will receive friends on Monday, February 23, 2015 from 10:30 to 11:00AM with services being held at 11:30 AM at Chapel of the Chimes Mortuary, 7924 N. 59th Ave, Glendale, AZ. Interment will follow at the National Memorial Cemetery of Arizona. Janet was cherished in life and will be missed always.
