He was born in Nacogdoches, Texas, on April 24,1937 to his parents Edgar and Ollie Mae Sullivan.
He was preceded in death by his parents Edgar and Ollie Mae Sullivan his wife June Collins Sullivan, and daughter Linda D. Perrin.He is survived by his two daughters Patricia Tucker and Carolyn Brown and four grandson Nathan L. Cook, Jeremy B. Cook, David W. McNeely Jr, and Thomas Wilburn McNeely. One great grandson David W. McNeely III, and three great granddaughters, Reanna N. Cook, Aubrey N. Cook and Josephine McNeely.
Memorial service arrangements are under the direction of Funeraria Del Angel in Grand Prairie Texas on January 16th 2016.
