Born July 19, 1961 in Indianapolis, he was the son of William Oscar Chadwick and Nina May (Sullivan) Collins.
Thomas had worked several years as a construction contractor and at IAC.
He is survived by his daughter Kayla Hensley (fiancée’ David Kaiser); siblings, Mark Ballard, Nina May Parsley, and Charles Morgan; two grandchildren, Landen and Alivia Kaiser; nieces & nephews; and special friends Daniel Sparks and Stacy Sparks and their families.
He was preceded in death by his parents and a sister, Bobbie Jo Schaefer.
A candle light vigil will be held in Thomas’ memory at a later date.
