Passed away quietly on October 12, 2014 in his 74th year, surrounded by his family at St. Joseph's Hospital. Beloved companion of Madeline Piper. Cherished father of Leslie (Ron), Cathy (Warren), Kelly, Jim (Heather) and John. He was a loving grandfather and great grandfather and will be missed by his brothers, sisters, nieces, nephews and friends. A special thanks to the doctors, nurses and therapists at St. Joseph's Hospital for their care. Keeping with Dave's wishes, cremation has taken place. In lieu of flowers, donations to St. Joseph's Hospital's Respiratory Clinic or to a charity of choice would be appreciated by his family.
