Cindy Michelle Slater Wortman, 41, passed away on Sunday, August 20, 2017. Visitation will be held on Wednesday, August 23, 2017 from 10:00 AM to 12:00 PM at Lakewood Funeral Home with funeral services to follow in the funeral home chapel.
Cindy was a wonderful woman with an outstanding free spirit. She enjoyed the outdoors along with many other things such as writing, poetry, music and dancing. Cindy loved her children incredibly. She will forever be remembered in our hearts and dearly missed by so many.
She is preceded in death by her father, Carl Walker Slater.
Cindy is survived by her children, Robin Rogers (Luke), Sabastion Wortman, and Courtney Nelson; mother, Kathern Slater; siblings, Mary Brown, Carolyn Slater Yates, and Carl “Cecil” Slater.
