Prior to retirement Mr. Church had worked in the automotive industry for many years.
He is survived by his wife of 37 years, Beverley Church, 4 sons, Michael Kohut, Joseph Kohut, John Church Jr. and William Church; two daughters, Karen Theilsen, Debbie Wojtowicz; three brothers, Doug Church, Gary Church and Larry Bray and one sister, Brenda Ernst, 6 grandchildren and 5 great-grandchildren.
A memorial service will be held on Thursday, October 22, 2015 at 11:00 AM at Hardage-Giddens Greenlawn Funeral Home, 4300 Beach Blvd. Jacksonville, Florida,32207
