

Dr. Olazagasti was born on December 25, 1946 in Santruce, Puerto Rico and graduated from the University of Puerto Rico Medical School. He completed his residency in Family Practice at Madegan Army Medical Center, Fort Lewis, Washington and was honorably discharged as Major in 1977.
He practiced Family Medicine in Latham, NY from 1977 to 1991 and was part-time Assistant Professor at Albany Medical College. Dr. O served as Physician Hospital Surveyor for the NYS Department of Health. He was Chairman of the New York State Board of Medicine and New York State Board of Medical License Restoration/Moral Character Panel and New York State Patient Occurrence & Tracking System (NYPORTS). He was also Advisory Board of Welch-Allyn. Dr. O was Medical Director of Saint Mary’s Hospital, Amsterdam, NY, North Jefferson Health Systems in Watertown, NY. Vice President of Medical Affairs with Samaritan Medical Systems, Watertown, NY and also at Benedictine Hospital, Kingston, NY. He was President of Olazagasti Healthcare Consulting Services, LLC and provided consultative and management services to healthcare and educational institutions in the U.S. and abroad. Dr. O was Consultant Dean for Academic Affairs at the American University of Antigua School of Medicine.
Dr. Olazagasti was predeceased by his first wife, Patricia Wilsey and his parents Adelaida and Rafael Olazagasti.
Survivors include his loving wife, Christine of Jensen Beach, FL, son Rafael & his wife Tonya of Baltimore, Maryland, stepsons, George and Paul Winters, brother Roberto and his wife lleana of Albany, NY, grandchildren, Rafael, Bella, and Lily Anne, nieces Aileen and her husband Steven Nicole, Drs. Alxa and her husband Allen Alvaraz and his nephew, Roberto Olazagasti and Lew Breckenridge and his great nieces and nephews.
He semi-retired to Florida in 2010 and began enjoying boating and learning to play golf. His hardy laugh and zest for life will be sorely missed.
Visitation will be held on Friday March 9, 2012 from 3:00 to 9:00PM at Babcock Funeral Home in Ravena, NY. A Mass of Christian burial will be on Saturday March 10, 2012 at 9:00AM at St. Patrick Catholic Church in Ravena, NY, a burial to follow at St. Patrick Cemetery in Ravena, NY.
