

DANIEL MICHAEL HAZEL, age 63, of Collierville, TN, passed away Sunday, September 2, 2012, at Baptist Memorial Hospital, after a well fought battle against cancer. He was surrounded by his loving family as he went to be with his Lord and Savior, Jesus Christ. Born on September 1, 1949, Dan was raised in Cleveland, Ohio and graduated from Cleveland State with a Bachelor’s degree in Marketing and Baldwin Wallace College with a Master’s degree in Business Administration. He was a veteran of the U.S. Navy. Dan worked for 17 years for True Temper Corporation. The last 17 years he worked for Aluma-Form Inc. Before going out on disability, he was President and CEO of Aluma-Form Inc. He also served on the board of director’s for Aluma-Form Inc. He has been a member of Our Lady of Perpetual Help Church since 1982 and a member of the Knights of Columbus. He was an avid football fan of the Cleveland Browns and the Auburn Tigers as well as a baseball fan of the Cleveland Indians. Dan was a wonderful husband for 41 years, an amazing father and grandfather. His warm heart and laughter will always be remembered by his family and friends. He is preceded in death by his mother, Bessie Lavelle, and his father, George Hazel, of Cleveland Ohio. Survivors include his wife, Lucy Hazel; his daughter and son-in-law, Lisa Hazel Perniciaro and Brady Perniciaro of New Orleans, LA; his grandkids, Ashley, Lauren, and Nathan Perniciaro of New Orleans, LA; brothers, George Hazel (Claire) of Cincinnati and Ed Hazel (Tina) of West Lake, Ohio; many nieces and nephews. Visitation will be held at 10a.m. on Saturday, September 8th at Our Lady of Perpetual Help Catholic Church in Germantown, TN. We will be having a Memorial Mass following the visitation at 11a.m. In lieu of flowers, the family requests that memorials be made to St. Jude Children’s Research Hospital.
