Jaqueline Fowkes Maxfield, loving wife, mother, grandmother and loving friend to all died peacefully at home on Saturday morning, May 19, 2018 after a valiant courageous battle with MS and breast cancer. Born on February 20, 1940 to John W. and Jean Larson Fowkes. Married the love of her life Dahl J. Maxfield on June 2, 1960 later sealed in the Salt Lake Temple on May 2, 1962.
Jacque was a devout member of the Church of Jesus Christ of Latter Day Saints and served faithfully in many positions. She had many interests including reading, cross-stitching, sewing, traveling and being with her family.
Jacque leaves behind her loving husband of 58 years, sister Suzanne (Bill) Nesbitt, four children Melanie (Rory) Levorsen Troy D (Tracy) Maxfield, Jon (Beverley) Maxfield and Roger (Marymaria) Maxfield, 13 grandchildren and 11 great-grandchildren. Proceeded in death by her parents.

A viewing will be held on Wednesday, May 23rd from 6:30PM to 8:00PM. Funeral Services will be held on Thursday May 24th at 11:00AM with an additional visitation from 9:30AM to 10:30AM, prior to the service. Both the visitation and the funeral service will be held at Imperial 1st Ward 2738 South Fillmore Street (1540) East, Salt Lake City, Utah.
