John Louie (Jack) Fornari was born January 11, 1944, to Clarice Barazzuol and Luigi Fornari. He had two children, Cindy and John, from his first marriage to Astrid Lammerts and three children, Paul, Crystal and Glenyce, from his second marriage to Ruth Blackmore. He also had 13 grandchildren.
Jack was diagnosed with metastatic cancer this past year and died peacefully on November 28, 2024. He spent his final days in the capable, kind and gentle care of the North Shore Hospice surrounded by his loving children, his sister Louise and cherished friends. Predeceased by his wife, Ruth in 2011, Jack will be missed by his children and grandchildren. He will especially be missed by his three sisters who so enjoyed their weekly siblings zoom calls over the last five years
