Passed away at Ottawa General Hospital on Monday, February 27, 2012 at the age of 81 years. Retired statistician with Statistics Canada and Indian and Inuit Affairs. Born August 4, 1930 in Blaine Lake, Saskatchewan, predeceased by his parents Fred and Anna (nee Cynkaluk) Zayachkowski, and brother Paul. Survived by brother Alex, sons Boris and Mark, granddaughters Tamara and Eva, and many cousins, nephews and one niece. Memorial service (Panakhyda) at 7:30 p.m. on Wednesday, February 29, 2012 at McEvoy-Shields Funeral Home, 1411 Hunt Club Road, with visitation from 6:30 – 9:00 p.m. Funeral service on Thursday, March 1, 2012 at 9:45 a.m. in Assumption of the Blessed Virgin Ukrainian Orthodox Cathedral, 1000 Byron Ave., followed by burial at the Parish Cemetery on Merivale Road. Memorial donations may be made to Assumption of the Blessed Virgin Ukrainian Orthodox Cathedral.
