CIESLIK, Adam Matthew, 15, of Surprise, Arizona, passed away on February 04, 2019. He is survived by his beloved family, his mother, Katarzyna H. Bialek Cieslik; father, Jerry C. Cieslik; and brothers, Patrick Cieslik and Daniel Cieslik. A Visitation will be held on Friday, February 08, 2019 from 6:00 PM to 8:00 PM at Surprise Funeral Care, 16063 W.
