Joseph J. Cupchak Jr., 55, of Surprise, Arizona, passed away on August 2, 2014. He is survived by his loving wife, Bernadette Brecht, mother-in-law, Shirley Brecht, aunt, Carol Hartman, mother, Dolores Cupchak, father, Joseph (Carol) Cupchak Sr., brother, Steven (Beth) Cupchak, cousins, Hank Hartman and Holly Harris and friend, Patricia Sweet. Jay’s family would especially like to thank all those who offered their love and support throughout his illness. A memorial gathering will be held on Friday, August 8, 2014 from 5:30 P.M. to 8:00 P.M. at Surprise Funeral Care, 16063 W. Bell Rd., Surprise, AZ 85374.
