Patricia Ann Kowalski was born October 24, 1949 in Chicago to Conrad and Ellen (nee Anderson) Hoffman. She died November 21, 2018 at her home in Arlington Heights.
Patricia was a registered nurse, specializing in geriatric nursing for 40 years.
Patty is survived by her siblings Susan (late Gene) Folkes and Michael (Kathy) Hoffman; her nephews Sean (Erika), Eric (Sarah) and Daniel (Patricia) and by her great niece Margaux and great nephew Lincoln.
Patty is preceded in death by herhusband, Dennis Kowakski; brother Peter (Beverly) Hoffman and by her parents. A memorial service will be held at a later date.
