A Celebration of Life will be held at McConnell Funeral Home on Saturday, July 29th, at 1pm, with Father Joy Chalissery officiating. The family will begin receiving friends at 12pm.
In lieu of flowers, the family has requested that donations be made to American Cancer Society, or your favorite charity.
Mr. Polka is survived by his son, Ted Evans (Shelli); daughter, Tracy Kozack; brother, Jimmy Polka (Joyce); grandchildren: Evan Innerarity (Jessica Bolan), Myka Kerr (Colin), Tara Schneider (Jason), Torrin Mallory (Jennifer), Dexianna Evans and Ben Dickson; great-grandchildren: Aydon Kerr, Joylin Kerr, McKenzie Kerr, Scarlet Schneider, Halle Schneider, Pierce Schneider, Tevin Mallory, Josiah Rector and Rosely Rector.
Preceding him in death is his wife, Julieanna Joy Polka; daughter, Tonya Dickson; brother, Paul Polka; and son-in-law, Edward Kozack.
