Phyllis M. Costa, age 72, was born in Chicago, Illinois on December 23, 1952, and went home to Heaven on Wednesday, April 30, 2025. Phyllis was the dear daughter of the late John & Rose Costa; and dear friend of her special caretaker, Beata Kuzebski.
Phyllis's Visitation will be on Wednesday, May 7, 2025, at 10:00 AM; immediately followed by her Celebration of Life Service at 11:00 AM at Montclair-Lucania Funeral Home, 6901 West Belmont Avenue, Chicago, IL 60634.
