Jack is survived by his Daughter Wendy and her husband Charlie Snellgrove of Clearwater, Jeff Brookins and his wife Joanne of Clearwater, Mike Brookins of Palm Harbor and Steve Brookins and his wife Sherri of Tarpon Springs, 8 Grandchildren Heather Manter, Holly Arbogast, Joe Brookins, Julie Brookins, Kevin Brookins, Philip Brookins, Lindsay Brookins and Matt Brookins, 2 Great Grandchildren Braxton Arbogast and Mason Arbogast. Jack was predeceased by one son Kevin Brookins in 1994.
A Celebration of Life Gathering will be held at the Sylvan Abbey Funeral Home on Monday December 1, 2014 from 6 to 8 PM. A Chapel Service will be held on Tuesday December 2, 2014 at 11 AM at the Sylvan Abbey Funeral Home will burial to follow at Sylvan Abbey Memorial Park.
