

John Frank Perez, D.O., FACOS, 72, of Worthington, died Friday, February 17, 2012 after a lengthy illness. He was born April 13, 1939 in Atlantic City, New Jersey and was a graduate of Holy Spirit High School. Jack attended Seton Hall College, went on to Kansas City College of Osteopathic Medicine, and completed his internship and residency in Columbus at Doctor’s Hospital where he was an urologist and general surgeon. Jack was the past president of the American College of Osteopathic Surgeons, Chief of Staff at Doctor’s Hospital, and one of the founders of the American Kidney Stone Management. He was the recipient of the Orel F. Martin Distinguished Service Medal, the highest award bestowed by the ACOS. He was a boating and fishing enthusiast and loved spending time at his residence in Florida on his boat (Pura Vida). Preceded in death by his parents, John Frank Sr., MD and Lydia Perez. Jack will be missed by his children, John (Sandra) Perez, Mark (Sandy) Perez, Jennifer (Chris) Albrecht; grandchildren, Alexandra, Samantha, Charlotte, Luke, and Sara; longtime companion, Lexie Tyus and her daughter, Sabrina Tyus-Hayes; and brothers, Robert Perez, DPM, Stephen Perez, James Perez, DO , and Michael Perez, DPM, and their families. Family will receive friends Wednesday, February 22, 2012 from 2-4 and 6-8 p.m. at SCHOEDINGER WORTHINGTON CHAPEL, 6699 N. High St., Columbus (Just south of I-270). Mass of Christian Burial will be held at 10 a.m. Thursday at St. Peter Catholic Church, 6899 Smoky Row Rd., Columbus, Rev. Justin J. Reis, Celebrant.
