Teresa passed away on Wednesday, January 27, 2010 in Louisville.
Teresa was a member of 600 Cerebral Palsy Club, Easter Seal Booster Club and Open Door Baptist Church.
Survivors include her mother and stepfather, Mary Lou and Wayne Klein; father, James E. Collins; brothers, Douglas and Dennis Klein.
