

William Form died in Upper Arlington on Saturday, October 17 at the age of ninety-eight. Born in Rochester, New York in 1917, he became an emeritus professor of sociology at three Midwest universities: Ohio State, Illinois at Urbana-Champaign, and Michigan State. He was preceded in death by his first wife, Mildred Cottone and by his daughter Cathy Sternberg. He is survived by his wife, Joan Huber; his daughter Helen Land; his step-children, Nancy Rytina and Steven Rytina; nieces, Evelyn Bovenzi, Maribeth Klobuchar, and Fern Myers and nephews, Anthony Form and Ron King; and by six grandchildren and two great-grandchildren. A Memorial Service will be held 2:00 pm Saturday, December 12, 2015 at Trinity Episcopal Church, 125 East Broad St., of which he was a longtime member. The Rev. Richard Burnett will officiate.
