Carolyn J. Hoffman, 79, passed away Thursday, July 5, 2012. She was born on June 9, 1933 in Bourbon, Indiana. She was a member of the Waynedale Methodist Church. Surviving family include her children, Rebecca (Richard) Olry, Steven Hoffman both of Fort Wayne, Robert (Chandra) Hoffman of Beavercreek, OH, Keith Hoffman of Indianapolis, IN; 3 grandchildren, and 4 great grandchildren. A funeral service will be 11:30 am on Tuesday, July 10, 2012 at Elzey-Patterson-Rodak Home for Funerals, 6810 Old Trail Road, Fort Wayne with visitation one hour prior. Burial will be at Covington Memorial Gardens. Memorials may be made to Cancer Services of Northeast Indiana, League for the Blind, or American Heart Association.
