He was born July 28, 1939, in Paulding, the son of the late Elizabeth M. (Miller) and Floyd A. Cook Sr.
He is survived by two brothers, Norman (Myrna) Cook, Paulding, and Stanley (April) Cook, Ayersville; and nieces and nephews.
He also was preceded in death by a sister, Sarah; and brothers, Harold “Bud” and Robert Cook.
A private family memorial service was conducted at 2 p.m. Saturday, July 15, 2017, at Den Herder Funeral Home, Paulding. He was laid to rest in Greenlawn Memorial Park, Fort Wayne, Ind., where his mother and father are interred.
In lieu of flowers, the family suggested donations be made to a charity of the donor’s choice.
