Jay Patrick Hoffman, 59, died Sunday, March 13, 2011 at St. Joseph’s Hospital. Mr. Jay Patrick Hoffman was born in Brazil, Indiana and graduated from Van Buran High of Clay County. He has two children by Mrs. Sharon Lee (Maxedon) Farrell, a son Mr. Jay J. Hoffman, and daughter, Mrs. Shoshannah Atwell. He also has step children from his second wife, Gloria Frye, whom is deceased; and they are Danny, Tracy, Carman, and Chad Frye of Indianapolis, Indiana. Also living are his eleven grandchildren who live in Indiana, Tennessee, and Georgia.
Graveside Service: 3:00 p.m. Thursday, March 17, 2011 at Greenwich Cemetery.
