In her work life, Charlie was known as the bubbly smiling face that greeted hundreds of patients at the Libbey Memorial Physical Therapy Clinic and McFarland Eye Center for many years. She took great pride in making sure every patient had a pleasant experience. If you had the pleasure of knowing Charlie in her personal life, then you know she could light up a room with a smile. She had a way of making sure every person she encountered felt special in some way. Charlie was friend to all. She had a love for live music, dancing, shopping, slot machines, adventures, and did we mention shopping? Gram was a hopeless romantic and loved her family dearly.
She experienced a series of strokes from June 2021 until December 2021 which eventually led to paralysis of the soft palate and other complications. Charlie's wishes were to have her medical conditions remain private and within immediate family during these times. She left this world peacefully with family at her side.
She was preceded in death by her husband, James Robert McIntosh; her parents, John Dale and Erlene Valdez; and her sister, Tambra Valdez Serrano.
Survivors include her daughter, Tiah Frazier of Hot Springs, AR; her grandchildren, Jessica (James) Kotarek of Bryant, AR, Kylie Frazier of San Antonio, TX and Justin Frazier of Bentonville, AR; her great grandchildren, Kenzie, Karter, and Becca Kotarek; her siblings, John (Barbara) Dale of Hot Springs, AR and Jeanne Withem of Tuscon, AZ; other family members include Michael Frazier of Bentonville, AR; and numerous aunts, uncles, nieces, nephews, and cousins along with a multitude of bonus kids and lifelong friends.
A special thank you to Dr. Kevin Hale, National Park Medical Center, Arkansas Hospice, Elder Estate Protection of Arkansas, and Gross Funeral Home.
