He leaves to cherish his memory, father, Carl Middlebrook Jr, mother, Ruby Middlebrook, brothers, Derrick Middlebrook, Darnell Middlebrook (Danielle), Dewayne Middlebrook, Eric Middlebrook (Jessica), sisters, Carla Middlebrook, Lashana Banks (Jerry), Donnisha Banks, Naffie Banks (Joe), Yolonda Middlebrook, Darlene Middlebrook, Tonisha Middlebrook, Jazmine Johnson and a host of extended family and friends.
His visitaton will take place on Saturday, June 14, 2014, 11am- 1pm, with the Homegoing Celebration commencing at 1pm at Mt Calvary Apostolic Church. His burial will take place in Sutherland Park Cemetery.
