Forrest passed away peacefully at home following a lengthy illness with his wife, Sally, at his bedside.
Forrest was born in Kansas City, Missouri. His father did government work and they moved frequently when he was growing up, so he lived in several different states eventually ending in Louisville, Kentucky. He was a veteran of the Korean War, serving four years in the Air Force.
He met his first wife, Sonnie Collini, in Louisville and they were married in 1956 and had two girls, Terri and Vicki. Forrest moved his family to Jacksonville in 1964 where he worked for State Farm Insurance until his retirement in 1995 after 31 years. Sonnie suffered a short illness and passed away in 1986.
He was introduced to his second wife, Sarah (Sally) Carlson by a mutual friend in 1988 and they were married in 1990. Sally had two sons, Michael and Brian from a previous marriage.
Forrest and Sally enjoyed traveling, making several trips to Europe, cruises and traveling across the United States. He enjoyed getting their combined families together to celebrate the holidays and birthdays.
Forrest is survived by his wife, Sally, his daughters Terri Roberts (John), Vicki Miller (Randall), stepsons, Dr. Michael Carlson (Patti) and Brian Carlson (Penny). He also has ten grandchildren and eight great grandchildren
He was loved by all and will be greatly missed.
There will be a Memorial Service on Saturday, September 12, 2020, at Hardage Giddens Funeral Home, 11801 San Jose Blvd. at 10:00 am.
