Phyllis was born on August 19, 1939 in Springfield, IL to Ethel (nee Oberman) and David Levine. She is predeceased by her parents and survived by her husband, Eugene “Gene” Siegel; son Edward “Ed” Siegel (Emee); daughter, Michele Berkowitz (David); sister, Jacqueline “Jackie” Pearlmutter (Norman); two brothers, Stanley Levine (Nancy) and Gerald “Jerry” Levine (Jeanette); and five grandchildren, Megan Siegel, Kristen Siegel, Adam Berkowitz, Aaron Berkowitz, and Rachel Berkowitz. Services will be held at 9:00AM on Monday, August 16, 2021 at King David Memorial Chapel and Cemetey, 2697 E. Eldorado Lane, Las Vegas, NV. King David Memorial Chapel handled the arrangements.
