She was preceded in death by her parents, Grace and Lucien Hornback; husbands James Puckett and James Lewis.
She is survived by her special friend, Regina Cox (Ken); cousin David Brown (Connie); and stepson Mark Puckett.
Visitation for Bernice will be held from 3-8 p.m. Monday at the Arch L. Heady & Son Funeral Home at 7410 Westport Road. A memorial service for Bernice will be held at 11 a.m. Tuesday in the chapel of the funeral home. Burial will be in Red Hill Cemetery in Hodgenville, KY. There will be no graveside services.
