

Carolyn was born on November 1, 1948 in Centertown, Kentucky to the late Wendell C. Chancellor and Iva Fern Wells. She is also preceded in death by her husband, Terrance A. Mohn; and her three brothers, Michael David Chancellor, Stephen Wendell Chancellor and James Dale Chancellor.
Carolyn graduated from Durrett High School in 1966. She moved to San Diego, California where she met and married the love of her life Terry Mohn on November 3, 1967. Their first child Donna was born in San Diego and the couple decided to move back to Kentucky for support and family help. They moved back to Carolyn's home state and city of Owensboro then eventually relocated to Louisville and with their second child Jason and started their beautiful life together.
Carolyn enjoyed watching her favorite shows, Wheel of Fortune and 1970's Price is Right. She loved animals, her cats and dogs over the years brought her so much joy! Most of all, Carolyn loved spending quality time with her family, her greatest joy was watching her grandchildren play sports and cheering them on the sidelines. She loved watching her grandson Grayson play baseball who plays for Trinity High School, and just committed to playing for the University of Kentucky. Grayson's journey and commitment to play brought tears to her eyes in the last month. Carolyn was a warm and loving soul, who will be dearly missed by all who knew her.
Left behind to cherish her memory are her two children, Donna Willoughby (Mark) and Jason Mohn (Heather); her three grandchildren, Alyson and Jacob Mohn and Grayson Willoughby; siblings, Sharon Ruth Servos, Patricia Fern Nishimori and Scott Anthony Stengel (Laura); and loving nieces, nephews and friends.
A visitation for Carolyn will be held Friday, September 6, 2024 from 4 p.m. to 8 p.m. at Evergreen Funeral Home, 4623 Preston Highway, Louisville, Kentucky 40213. A service to honor the life of Carolyn will be held on Saturday at 10 a.m. at the funeral home. Carolyn will be laid to rest at Evergreen Cemetery, alongside her beloved late husband, Terrance A. Mohn.
