He is preceded in death by his three brothers, Edward, Charlie and Delmer.
He is survived by his two sons, Tim (Trina) Collinsworth and Gregory (Karen) Collinsworth; daughter, Jeannie (Glenn) Becraft; six grandchildren; four great grandchildren; two sisters, Pauline (Meryl) Nickel and Marie (Homer) Blevins.
Visitation will be held Wednesday, July 24, 2019, 11am-1pm at Breitenbach-Anderson Funeral Home, 517 S. Sutphin Street, Middletown, Ohio 45044. Funeral Service will follow at 1pm with Pastor Danny Griffith officiating. Burial immediately following at Butler County Memorial Park.
