

He is preceded in death by his grandparents, Edith and Elmore Boudreaux, Leroy and Agatha St. Germain, Anthony Fazzio, Elden Trosclair, and George and Marian Thibodeaux, his brother, Corporal Michael Middlebrook, sister-in-laws Holly, Melissa and Michelle Faucheaux, and nephews, Colby and Kade Boudreaux.
He is survived by his loving wife Brandy, their son, Rhett, his parents, Herman and Rose Middlebrook, and Curtis and Connie Boudreaux, parents-in-law, Mike and Darlene Faucheaux, siblings, Mandy Lovett (Michael), Jill Cadow (Eddie), Jason Smith (Elizabeth), and Michele Harris (Adrian), and grandparents, Carolyn Fazzio, Enola Trosclair, and Yolanda Smith. He is also survived by many nieces, nephews, and a large extended family, all of whom loved him completely and fully.
Curtis loved the outdoors, hunting, boiling seafood, and having parties. He was happiest being on the ball field around the diamond coaching his team, the Rivercats. He was the best uncle to all of his nephews and nieces, but his whole heart was devoted to being with his love, Brandy and his best friend and son, Rhett.
A funeral service will be held Wednesday, April 13, 2022 at St. Charles Borromeo Church, 13396 River Road, Destrehan, LA. Visitation will be held from 10 AM to 12 PM with a mass to begin at 12 PM. Interment will follow at St. Charles Borromeo Cemetery. There will be a celebration of his life following the services at Cypress Lakes Country Club for all who want to attend.
