She was born June 30, 1928, in Fairfield, Alabama. She celebrated her second birth in Christ when she was 35 years old. Hazel was a loving and caring Mother, and was loved by all who met or knew her, especially her children. Hazel was a nurse technician with the Baptist Health System for many years.
She is preceded in death by her Father, Robert L. Freeze, and Mother, Mary Turner Freeze, and her granddaughter, Lyndie Percer, and stepson, James M. Leslie.
Hazel is survived by her sister Doris Mahan; children Thomas Percer, Carol Agre (Ed), Scott Percer (Jan), and stepson John Leslie (Loretta). Grandson, Jeffrey Agre (Dana), and great-grandson Henry Agre and great-granddaughter Eleanor Agre. Step Grandson, Julius Rickles.

A service will be held at Southern Heritage Funeral Home and Cemetery, located at 475 Cahaba Valley Road, Pelham, AL, 35124, US., on Thursday, December 4, 2025, at 12:00 pm, with a graveside to follow at Elmwood Cemetery.
